Want to get testicular exam. Worried dr. will laugh at my ?

k so my right testicle hurts so i want to get it checked just incase. Im 20 and because im heavier set there is fat on on pubic area that makes my only 1.5 inches when flacid. I literally am a grower cause it goes to 5.5 inches (yes i measured) when erect but i cant exactly show that to the dr. My question is how professional are these guys. I dont want some a$$ laughing at me in the middle of the exam.
